Welcome to the virtual labor doula support page. Thank you for taking the time to chat with me before reading this page. Below are all the documents you need for your labor doula support.
As we spoke about, this virtual labor doula support will be free to you in exchange for you, your physician, and one of your support staff (nurse, tech, etc.) filling out the paperwork below for my certification.
First, I will need you to click the link below and complete the client release form for CAPAA (Childbirth and Postpartum Professional Association) and email it to ashley@ashleyrobinsonibclc.com with the subject “Client Release Form.”
*Note: you will need to download the form and complete it in adobe reader, or you can open it on your phone, screenshot it, and sign with an application.

Labor Doula Support Forms
STEP 2: Expectant Mom and Partner
Birth Plan and Medical History
Click the link and download the pdf. Complete the birth plan and medical history form, then email it to ashley@ashleyrobinsonibclc.com before our prenatal meeting.
STEP 3: New Parents
Doula Evaluation for Parent(s) to complete
This is a CAPAA form that will need to be completed by the parents after the birth of their new baby. The form is to evaluate the doula support that was provided.
STEP 4: Delivering Physician/Midwife
Delivering Physician/Midwife Evaluation Form
This form must be completed and signed by your healthcare provider after the delivery. You can print this form, or you can send this website to your healthcare provider. With past experiences, it is usually easier to print and take to the hospital.
STEP 5: Nursing Staff
Nursing Staff Evaluation Form
This form must be completed and signed by one person from your nursing staff who assisted during your delivery. Again, you can print this form, or you can send this website to them to complete.
